Shout out to Jill M Ham at the Dyslexia Center of Georgia for this wonderful sample letter! Don’t be intimidated because you have to write your concerns down. You do not have to be an expert in learning disabilities or use technical words. Just describe what you have noticed. “John does his homework each night and we put it into his backpack but he forgets it’s there and has gotten zeros for the last 2 weeks because he doesn’t turn it in.” That’s a good description of your concern. “Cassie has trouble identifying letters. Last night she couldn’t read the letter C even though it’s the first letter of her name.” Perfect!
One hint – change the state of Georgia and remove the reference to the state regulation unless you are in Georgia but the other citations are for the federal law and they apply no matter where you give.
